- DVT/PE treatment: 10 mg twice daily for 7 days, then 5 mg twice daily; for prevention of recurrence, 2.5 mg twice daily
- Swallow with water, with or without food, at the same times daily (e.g., morning and evening). Crush and mix with water, apple juice, or applesauce if unable to swallow whole; use within 4 hours. Can be given via nasogastric tube. Do not stop without doctor’s advice.
